Before Your Visit
If you are new to the Center for Sight, our team will review your insurance information with you at the time you call to schedule your appointment. You can download a Patient Information Form and a Health History Questionnaire below. Please fill these forms out as completely as possible and bring them to your first appointment. If you prefer to fill out forms in the office, please arrive 15 minutes prior to your appointment time.
It is always best for you to check with your insurance company to verify your medical coverage. Remember that it is ultimately the patient’s responsibility to know your insurance benefits. Any fees your insurance does not cover, you will be responsible for. You may need to see your primary care physician prior to your visit to our office.
You may want to bring your sunglasses with you to your appointment. As a part of your examination, your pupils may be dilated. With dilation, most people experience light sensitivity and difficulty with close focusing (such as reading). Some individuals also have difficulty seeing in the distance. The effects of the dilation can last up to several hours.

Center for Sight is a patient-focused ophthalmology practice providing comprehensive eye care. This means we provide care for patients of all ages with a complete array of eye care needs. Patients needing additional care from another specialist will be referred to after consultation with Dr. Liang. Most surgical procedures can be performed in our ASC (Ambulatory Surgical Center), which is located at the back entrance of our building.
